Death Records
The City of Canandaigua has death records of anyone who died within the City of Canandaigua. Certified copies may be obtained by parent, spouse, child or sibling by following the procedure listed as follows:
Apply in person at the City Clerk's Office during office hours, 9 am to 4 pm, Monday through Friday. The fee for a certified copy is $10 per copy requested. or send a written request using the form below which should include the following:
- Decedent's Name
- Date of Death
- Your relationship to decedent
- Copy of identification - such as driver's license
If applying for the certificate by mail, please include a Money Order for $15 per copy requested, your identification and the check can be made payable to the City of Canandaigua.

Important Note
Family relationship may need to be verified - for example, a daughter requesting a copy of her father's death certificate would also need to provide a copy of her birth certificate showing her father's name.
Death records are also available to attorneys or for any lawful legal purpose which must be verified through documentation. Please reach out to the City Clerk for more information.

For your convenience, the application form below may be downloaded and printed. However, it is not possible to apply online. Applications and supporting documentation must be mailed or brought to the City Clerk's Office at City Hall. All original documentation received by mail will be returned with your requested paperwork.
